Please provide the following required documents for your initial appointment:
Intake Form
Copy of picture ID for all family members, 18 and older in the household
Proof of income (last 30 days) for family members, 18 and older in the household
Proof of physical address, for example, utility bill in your name
ADDITIONAL REQUIREMENTS IF APPLICABLE
Income and Expenses Form
Supplemental Security Income (SSI) award letter if applicable
Social Security Disability Insurance (SSDI) award letter if applicable
Proof of Child Support if applicable
Proof of crisis
Bank statement and transactions for past 30 days
Proof of lease agreement
Additional documents as requested by HCFS
ALL CLIENTS MUST PROVIDE ABOVE LISTED DOCUMENTS WITHIN 72 HOURS OF INITIAL APPLICATION IF NOT YOUR CASE WILL BE CLOSED. YOU CAN REAPPLY AFTER 30 DAYS WITH A NEW CRISIS.
Case Management includes, but is not limited to:
Assessment
Advocacy if needed
Establish and follow up on goals agreed upon by both case manager and client
All clients are assured confidentiality while working with a case manager. An Authorization to Release Information form signed by the client is needed for a case manager to discuss a client with other agencies.
Exceptions to the rule of confidentiality:
HCFS staff will notify any potential victim and the authorities if staff feels a client is likely to harm another individual
HCFS staff will report any suspected child abuse, neglect or molestation to protect minors
HCFS staff will report any suspected cases of elderly abuse or neglect
If HCFS staff determines that a client is suicidal or unable to care for themselves, the proper authorities will be contacted
Failure to comply with the program rules and/or instructions by the case manager or administrative staff will result in a suspension of services

Los servicios de gestión de casos incluirán, entre otros:
