Getting Started

Step 1 - Apply

Fill out the application below and a member of our staff will reach out to you

to discuss your case.

 

You will be requested to submit a copy of an identification card, proof of residency and proof of income.  Depending on your specific request, more documents may be needed.

Step 2 - Crisis Evaluation

Clients are given an appointment to meet with an HCFS case manager in order

to receive a comprehensive evaluation of their crisis.

Part 1 of a 2 part personal assessment will be given by your case manager.

Step 3 - Re-Evaluation

We reassess quarterly to confirm solutions are working.

Your case manager will contact you to complete part 2

of the 2 part assessment.
